If a student had a “White Card” and it is lost/stolen or destroyed, the student may be able to apply for a replacement card, dependant on the course code. Completion of the replacement white card form, 3 forms of identification and a copy of the statement of attainment showing the relevant unit code issued from a Queensland RTO, are required to complete the replacement process. Only units CPCCOHS1001A, CPCCWHS1001 and CPCWHS1001 are still valid and can be replaced (as a general rule, if your White Card was issued prior to July 2009, it is likely too old and no longer valid).
